Aging of the face is a natural process of the human body that can often result in undesirable changes. Collagen, the protein that gives skin its elastic and durable properties, begins to break down over time. Facial skin becomes thinner and less stretchy.

In addition, the ligaments of the face that hold the skin up in its high, youthful position begin to weaken.  This results in dropping or sagging of the skin.  The typical changes we see in the face are jowl formation along the jawline and skin laxity of the neck under the chin.  Other undesirable changes to the face can also occur from the aging process.

Fortunately, a wide variety of procedures -- both surgical and nonsurgical – exist to help turn back the clock on aging. We have the ability to rejuvenate your face so you can regain a natural, more youthful look.

Dr. Yamamoto’s long-standing philosophy with facelift procedures is to provide a more youthful look while keeping results very natural. He wants you, your family and friends to see a more refreshed, energetic version of you. They will notice you look great, but will not notice the surgery itself. You don’t have to worry about the over-pulled, over-filled look.

 

 

Mini Facelift

The “Mini RT” Facelift (Mini RT = mini recovery time) was developed as an alternative to the traditional facelift for those patients who are looking for facial improvement with minimal time away from their busy schedules. Ideal candidates for the mini facelift are individuals around the ages of 40-60 with early signs of aging.

The “Mini RT” provides the changes and benefits of a traditional facelift, but with a minimal amount of downtime and at about half the cost. The procedure addresses the lower third of the face and improves the jawline and jowl area, as well as the neck.

The “Mini RT” is unique compared to other mini facelift techniques, in that it uses the techniques employed by traditional facelifts to lift and reposition the underlying facial musculature or SMAS (superficial musculo-aponeurotic system). This is much better in comparison to the “Lifestyle Lift” which uses permanent suture to hold tissue in place or the “Feather Lift” which uses barbed suture to hold up the facial skin. The repositioning of the muscle layer deep to the skin in the “Mini RT” is what provides the most dramatic and most long-lasting effects and makes it the best alternative to a traditional facelift. In addition, this procedure can be done in conjunction with any other rejuvenation procedure, including eyelid surgerya browliftnose reshaping, or laser skin resurfacing.

The incisions in the “Mini RT” are hidden within the hairline, continue into the creases of the ear, curve just inside the ear itself, and then curve underneath the earlobe. All incisions have been specially designed to minimize any potential scarring. Most patients have difficulty finding the incision sites themselves once fully healed.

The “Mini RT” can be performed under light general anesthesia or local anesthesia with oral medications, depending on the preference of the patient. The procedure is performed at a local ambulatory surgical center and the patient will go home the same day.

Contemporary Full Facelift

Unlike facelifts of the past, the contemporary full facelift can be tailored to suit the individual needs of the patient. Depending on the areas of concern of the face and neck area, Dr. Yamamoto will create a customized facelift plan to maximize results and minimize recovery time.

A full facelift addresses all areas of the lower third of the face, including the jowls, jawline, neck laxity, and excess skin and fat. While the procedure does not stop the aging process, tightening the facial musculature, redraping the skin, and removing any excess skin and fat can help “turn back the clock” 10 to 15 years. In addition, this procedure can be done in conjunction with any other rejuvenation procedure, including eyelid surgery, a browlift, nose reshaping, or laser skin resurfacing.

Dr. Yamamoto begins the incision within the hairline above the ear. The incision extends into the natural creases in front of the ear, curves within the ear itself, curves around the earlobe to the crevice behind the ear, and then back into the hairline. This incision and closure of the area with special suture has been specially created to both minimize the chance of any noticeable scarring, as well as provide the most natural result.

Necklift

As the neck ages, the muscles of the neck become lax and more visibly prominent. Often “banding” can be seen along the front and sides of the neck, and excess skin can contribute to the loose, sagging appearance.  The youthful, smooth neck with a high, tight angle is lost and is replaced by the dreaded "turkey gobbler."

With the Necklift procedure, the sagging muscles of the neck are pulled up and held in place with strong suture to tighten the entire area.  This helps smooth the neck skin, eliminate visible neck muscles, and create a new sharper angle, giving back that more youthful appearance. The special sutures used in the procedure give the lift a hold that lasts many years. This procedure is commonly done in combination with a facelift to achieve full rejuvenation of the entire lower third of the face and neck. 

The incision the Necklift starts just under the earlobe and continues along the crease behind the ear.  It then curves and is hidden into the hairline.  This incision is designed to be inconspicuous and allows you to wear a ponytail without it being visible.

Browlift

As the face ages, the position of the eyebrows starts to drop into a lower position.  This can lead to a brow that appears heavy or tired. Lowering of the brow can also contribute to excess upper eyelid skin or difficulty opening your eyes.

An Endoscopic Browlift is performed to elevate the eyebrows and provide a more “awake” and more feminine appearance. This procedure unweights the brow and eyelids and is performed endoscopically with small incisions made within the hairline, without any visible scars. The surgery is done entirely with visualization from a small telescope that is passed under the forehead skin.  The brow is carefully lifted and fixed in place with a dissolving plate. 

The result is a brighter, smoother, more youthful appearance from a minimally invasive procedure. This procedure can be done in combination with other eyelid and facelift procedures.

Chin Augmentation

The chin is an often overlooked but important feature of the face that creates proper proportions or balance.  A small or “weak” chin can make a nose look too big, accentuate an overbite, or contribute to the formation of a double chin or loss of the neck angle.

A Chin Augmentation is performed to enlarge a small chin and recreate the proper proportions of the face.  The procedure is straightforward and can be done in the office under local anesthesia or in the OR in conjunction with other procedures.  A small incision is hidden just under the chin, and a flexible silicone implant is placed along the edge of the jawbone.

A small, conservative chin implant in a woman is used to feminize the face and improve the neck contour.  It can also dramatically improve the result of a rhinoplasty or facelift performed at the same time. 

A slightly larger chin implant in a man is a great way to create a stronger, more masculine facial profile.

Liposculpting

Excess fat or fullness under the chin is a common issue that many individuals deal with.  Fullness in this area can lead to the dreaded “double chin” or loss of neck contour.  Direct removal of the fat under the chin remains the gold standard to improve the overall shape and angle of the chin.

Liposculpting, or "liposuction" is most often performed with a facelift where a small micro-cannula is used to gently remove the fat immediately under the skin. This is done through small a small incision hidden under the chin.

This procedure can also be done on its own under local anesthesia in the office.  You can have fat removed from your neck and chin, walk out of the office, and enjoy the results immediately.
